Suddenly Different Body Odour: What It Can Mean and When to See a Doctor

Has your body started smelling different out of the blue, even though nothing else has changed?

A new “note” to your scent can be about far more than a few extra beads of sweat.

Many people put a change in body odour down to getting older or to “stressful days”. But the blend of skin, sweat and bacteria is extremely sensitive to internal changes. Sometimes it reflects harmless triggers such as food or medicines; at other times it can point to more serious disruptions in metabolism or organ function.

Why sweat itself barely smells

Sweat on its own is almost odourless. The familiar body smell develops when sweat meets bacteria. Your skin hosts billions of microorganisms, which break down components of sweat and release scent compounds in the process.

There are two main types of sweat gland:

  • Eccrine glands: found almost all over the body; they produce mainly water and salt and help regulate body temperature.
  • Apocrine glands: concentrated mainly in the armpits and groin; they release a secretion containing fats and proteins - an ideal food source for bacteria.

It’s the apocrine secretion in particular that creates a stronger odour once bacteria break it down. How noticeable this becomes depends on genetics, hormones, hygiene, clothing, diet and medication.

What ageing has to do with body odour

Over the years, the make-up of skin oils shifts. From around the age of 40, many people produce more of a substance called 2-nonenal, which is formed from certain unsaturated fatty acids.

2-nonenal is often described as slightly “grassy” or musty - and it can be difficult to remove even with water and normal soap.

This is part of a normal ageing process and isn’t a reason to panic. Creams with specific degreasing ingredients or body oils can help to soften the smell.

However, if your body odour suddenly becomes completely different - for example, like solvent, fish, ammonia, or an intensely sweet smell - it’s worth looking more closely.

Warning signs: when body odour may point to illness

In internal medicine, the smell of skin and breath is considered an additional clue during assessment. Certain odour patterns recur in particular conditions.

Fruity or like nail varnish remover: a sign of diabetes

With poorly controlled diabetes mellitus - especially when there is a marked lack of insulin - the body can no longer move enough sugar into the cells. It then relies more heavily on fat, producing so-called ketone bodies.

Some of these ketones, particularly acetone, are excreted via the breath and sweat. People affected may smell:

  • fruity-sweet, or
  • slightly like nail varnish remover / solvent.

This situation can progress to diabetic ketoacidosis - an acute medical emergency with nausea, vomiting, abdominal pain and rapid breathing. In that case, you need medical treatment immediately, and if necessary should attend A&E.

An ammonia or “urine” note: weakened kidneys

With severe, long-standing kidney impairment, the kidneys no longer filter waste products from the blood effectively. Urea builds up and some of it is excreted through the skin.

Skin bacteria convert urea into ammonia. This can cause a sharp, pungent, “urine-like” smell. Clinicians refer to this as a uraemic odour.

As a rule, these odour changes appear only at an advanced stage. Blood tests of kidney function (for example creatinine and urea) usually show abnormalities much earlier.

Musty-sweet odour: liver damage

In pronounced liver failure, metabolic by-products build up in the body because the liver can no longer break them down properly. Certain sulphur-containing compounds may then escape through the breath and skin.

Experts call the typical musty-sweet smell “fetor hepaticus” - some people say it resembles damp, mouldy rooms.

This sign also usually appears only once the liver is already severely damaged. In such cases, blood tests for liver function would typically have been abnormal for some time.

How diet and everyday life shape your smell

Much more often than serious illness, lifestyle factors sit behind a change in body odour. Some foods are particularly well known for it:

  • Garlic and onions: sulphur-containing compounds enter the bloodstream and are excreted through breath and sweat.
  • Cabbage vegetables and broccoli: can encourage a more intense, slightly sulphurous sweat smell.
  • Asparagus: changes urine smell for many people and, in some cases, also sweat.
  • Heavily spiced foods: especially with curry, cumin or chilli, can noticeably influence body odour.

These changes usually appear a few hours after eating and fade again when the foods are eaten less often.

When medication increases sweating

Many tablets don’t alter the smell of sweat directly; instead, they increase the amount you sweat. More sweat means more nutrients on the skin for bacteria - and that can make odour stronger.

Common triggers include, for example:

  • certain antidepressants (especially SSRIs),
  • hormonal treatments, such as during the menopause,
  • medicines that lower blood sugar,
  • some painkillers or anti-rheumatic medicines.

If heavy sweating begins suddenly after starting treatment, it’s worth discussing it with the prescribing clinician. Sometimes a small dose adjustment or switching to another medicine is enough.

When you should see a doctor about changed odour

The key question is whether the new smell is temporary or persists - and whether other symptoms appear alongside it.

Typical alarm signs that should be taken seriously

If you notice a new, unpleasant body odour and also have one or more of the following, don’t wait too long:

  • night sweats that soak the bed linen,
  • intense thirst and frequent urination,
  • unintentional weight loss,
  • pronounced exhaustion with no clear reason,
  • fever or a persistent feeling of being unwell.

In an internal medicine or GP setting, this is usually followed by a detailed history, a physical examination, and blood and urine tests. The odour change is simply the starting point for broader diagnostic checks.

What you can do yourself before you worry

If you’re unsure, a few straightforward steps can help first:

  • Observe for a week: when does the smell occur - after which foods and at what times of day?
  • Check clothing: synthetic fabrics can trap sweat; cotton or blended fabrics are often better tolerated.
  • Adjust hygiene: use gentle, pH-neutral washes, keep armpit hair trimmed, and change towels regularly.
  • Review your medication list: note any new additions and mention them at your appointment.

If the smell persists despite these measures, or feels completely unfamiliar, it’s sensible to get a medical opinion - not least to reduce uncertainty.

How smell is produced at all: a brief look at biochemistry

Odour molecules are usually small, volatile compounds that evaporate easily and reach the nose through the air. On the skin, they can form when bacteria:

  • break fats down into shorter fatty acids,
  • split proteins into sulphur-containing compounds,
  • metabolise sugar residues and produce alcohols or ketones.

Which bacteria dominate depends on moisture, the skin’s pH, hormone levels and care habits. Deodorants and antiperspirants influence this balance by reducing sweat or slowing bacterial growth.

Some people naturally have a different mix of bacteria. That helps explain why one person smells “neutral” after exercise while another smells strongly after only brief exertion - even with similar hygiene.

Risks of fragrance overload and misplaced shame

Many people try to mask unpleasant body odour with strong perfumes, deodorant sprays, or repeated showers. This can irritate the skin barrier, encourage allergies and disrupt the natural balance of the skin flora.

More troubling is when embarrassment stops someone from seeking medical advice. For instance, if fear of “awkward questions” leads someone to ignore a striking new smell, they could - in the worst case - miss a metabolic problem that is very treatable.

The body communicates very directly through smell. Not every change signals illness, but any persistent, unexplained shift deserves attention - and, when in doubt, a quick medical check.
